Informed Consent Form

Ethical Considerations in Data Collection for Quantitative Research

Ethical considerations are essential when conducting research involving human participants. Researchers must ensure participants are treated respectfully, especially during data collection. One key element of ethical research is obtaining informed consent, which protects participants’ rights and ensures their voluntary participation.

Informed Consent Form

An Informed Consent Form provides participants with the necessary information to decide whether to participate in a study. This form should be completed before data collection begins and must be signed by both the researcher and the participant, acknowledging their agreement to the study's conditions.

  • Institution: Gulaman National High School-X
  • Department: Senior High School

1. Purpose of the Research Study

You are invited to participate in a research study that aims to improve problem-solving skills in mathematics using the Singaporean method. The study will evaluate the effectiveness of this approach in developing speed and accuracy in solving math problems.

The study will address the following questions:

  • Is there a significant difference between learners' problem-solving skills in the pretest and posttest?
  • Is there a significant difference in learners’ self-assessment of problem-solving skills before and after the introduction of the Singaporean Method?
  • Is there a significant difference between the performance of the control and experimental groups?

2. Procedures

  • You will be asked to use a workbook prepared by the Math Society and attend math classes using the Singaporean approach.
  • You will also participate in a Focus Group Discussion (FGD) to provide feedback on the method.
  • Duration of Participation: October 2020 to January 2021

3. Possible Risks or Discomforts

No significant risks or discomforts are anticipated during this study. The implementation of the method will take approximately 12 weeks, and the interview will last for two hours.

4. Possible Benefits

The findings of this research will benefit teachers by expanding their teaching methods to improve students' problem-solving abilities. The study could also inform curriculum development in schools, helping both public and private school administrators enhance problem-solving skills among students.

5. Financial Considerations

There are no costs associated with your participation in this study.

6. Confidentiality

Your identity and any information gathered during this study will be kept confidential. The results will be published for academic purposes only. However, data collected may be reviewed by the research sponsor, study conductors, or research panelists.

7. Termination of the Research Study

Participation in this study is entirely voluntary. You may withdraw at any time without penalty.

8. Available Sources of Information

If you have any questions about this study, the researcher will provide answers.

9. Authorization

I have read and understood this consent form, and I voluntarily agree to participate in this research study. I acknowledge that I will receive a copy of this form and that my participation does not waive any legal rights.
